PHARMACY

 

- If you have any questions about medication call the Medicine Pharmacist (5-6150).

- If you need a drug stat, you can get it most quickly by picking it up yourself from the Pharmacy on Osler 2 next to the Osler elevators.

- IV push Medications that must be given by a physician and cannot be given by a non-ICU nurse include some narcotics, digoxin, diltiazem, adenosine, beta-blockers, and haldol.

- Discharge medications are written on the white prescriptions.  Narcotic scripts on pink narcotics scripts which you can ask the nurses for.

 

Pharmacy Orders

QAM = 8 AM, QD = 10 AM, QPM = 6 PM, QHS = 10 PM, BID = 10 AM & 10 PM, ONCALL = the next 6AM, Warfarin standard time: 6 PM.

Routine = starts at next administration time for that schedule

Nroutine = starts with a now dose, then continues on standard times

Orders for ID approval drugs received between 8AM and 11PM require either ID approval or a correctly-completed ID Approval Form.  The medications will not be sent without one of them.  The ID Approval Pager is on Pagerbox.  Drugs that require approval: Amikacin, tobramycin, Azithromycin (except weekly MAC prophylaxis), All third-generation cephalosporins, Fluconazole (except single dose treatment for vaginal candidiasis), Fluoroquinolones (gatifloxacin, ciprofloxacin, norfloxacin), Lipid/liposomal amphotericin (additional form required for ordering), pipercillin/tazobactam, Imipenem/meropenem, Vancomycin (also needs a special vanc form), all non-formulary antibiotics (nafcillin, Timentin, cefotaxime, Unasyn).
